Fees for Participants include participation in all scientific sessions,
printed material of the congress, an invitation to the Get-Together
Reception as well as lunches. Fees for participants will also include
a membership to ISSAM - the International Society of the Aging Male,
which includes a one year subscription to the Society Journal.

Payment of registration fees may be made by cheque payable

Please make sure that the name of the congress and the participant

and Amex). Payment will be charged in US$ according to the rate
of exchange to Euro on the day of payment.
OFFICIAL CARRIER
Czech Airlines is the official carrier for the 4th World
Congress on the Aging Male and will be offering reduced rates to the
congress participants. To qualify for these rates please approach
the CSA branch in your country with an official letter of invitation
or receipt of registration payment. These reduced rates will be available
from one week prior to, and one week after the congress.

VISA REQUIREMENTS
All foreign visitors to the Czech Republic must possess
a passport valid for at least 3 months. Participants requiring a
visa should apply in advance to consular offices of the Czech Republic
or diplomatic missions in their country, in order to avoid delay
in travel to the congress. Please note that the visa application
procedure can take up to two months.
CME CREDITS
The 4th World Congress on the Aging Male is accredited
by the European Accreditation Council for Continuing Medical Education
(EACCME) to provide the following CME activity for medical specialists.
The EACCME is an institution of the European Union of Medical Specialists
(UEMS), www.uems.be
The 4th World Congress on the Aging Male is designated for a maximum
of 24 hours of European external CME credits. Each medical specialist
should claim only those hours of credit that he/she actually spent
in the educational activity.
EACCME credits are recognized by the American Medical Association
towards the Physician's Recognition
Award (PRA). To convert EACCME credit to AMA PRA category 1 credit,
contact the AMA.
AIRPORT INFORMATION AND TRAVEL TO PRAGUE
International flights arrive at Prague's Ruzyne Airport
located 20 km from downtown.
- The private Cedok minibus shuttle links the airport and Namesti
Republiky, a downtown square near the Old Town. Shuttles run in
both directions every 30-60 minutes between 11:00 and 16:00 hrs.
CSA Airlines also run a regular and reliable bus service into
the center. The travel time is approximately 30 minutes
- Private taxis are available 24 hours a day. Approximate cost
is € 23 per taxi.
LETTER OF INVITATION
Upon request, the Secretariat of the Congress will send
a personal invitation to participants. This invitation is meant
only to help visitors to raise travel funds or to obtain a visa,
and is not a commitment on the part of the organizers to provide
any financial support.
LIABILITY
The Congress Secretariat and Organizers can't accept liability for
personal accidents, loss of or damage to private property of participants,
either during, or directly arising from the 4th World Congress on
the Aging Male. Participants should make their own arrangements
with respect to health and travel insurance.
CLOTHING
Clothing is informal for all occasions.
