PUBLIC HEALTH
PROGRAM/OPEN SOCIETY INSTITUTE-NEW YORK
BACKGROUND

The experience of dying has changed over the past several decades, with many
more people enduring prolonged deaths as a consequence of chronic and
progressive disease. Needless suffering-physical, emotional, existential, and
spiritual-too often accompanies these deaths, for both dying persons and their
survivors.
The purpose of the Palliative Care in Eastern Europe Initiative is to improve
the care of the dying.
SEVEN
PROGRAM AREAS WILL BE FUNDED

PROGRAM #1:
PALLIATIVE CARE
RESOURCE/TRAINING CENTER
(You may apply for a maximum amount of $75,000 a year for two with
potential for renewing for a third year) Capacity building grants to exemplary
institutions and/or organizations to serve as regional resource and training
centers for professional education in pediatric and adult palliative care.
PROGRAM #2:
PALLIATIVE CARE REGIONAL
EDUCATION PROGRAMS
(You may apply for a maximum amount of $50,000 for a two year period)
We will provide funding for regional educational programs for health care
professionals (physicians, nurses, social workers, pharmacists, pastoral care)
from more than one country and up to five different countries. Applications for
programs lasting from one day to one week will be considered.
PROGRAM #3:
PALLIATIVE CARE NATIONAL
EDUCATION PROGRAMS
(You may apply for a maximum amount of $20,000 for a two year period)
We will provide funding for national education programs for health care
professionals (physicians, nurses, social workers, pharmacists, pastoral care)
from within one country. Applications for programs lasting from five hours to
five days will be considered.
PROGRAM #4:
REGIONAL
MEETINGS OF POLICY MAKERS
(You may apply for a maximum amount of $20,000 for a two year period)
In order to influence the sustainability of palliative care in the region, we
will provide funding to bring together drug regulators and the appropriate
governmental and non-governmental leaders from more than one country and up to
five countries to discuss the financial, social, and legal issues related to the
integration of palliative care into public health policy.
PROGRAM #5:
PALLIATIVE
CARE SCHOLARSHIPS
(OSI will determine the budget for these scholarships and assist in
arranging placements.)
One to three month scholarships to support training of health care professionals
in end of life care at established palliative care and hospice centers.
PROGRAM #6:
PALLIATIVE CARE TRAVEL
GRANTS
(You may apply for a maximum amount of $2,000)
Travel grants will be provided to identified experts from the region to attend
and present at international congresses and symposia on palliative care.
PROGRAM #7:
EDUCATIONAL MATERIALS
(You may apply for a maximum amount of $5,000)
One of the most serious problems for professionals working in the field of
palliative care is the lack of information (either in English or in their native
language). We will provide funding for the translation of existing palliative
care educational materials.

ELIGIBILITY
CRITERIA

1. Applications will be accepted from the

University, medical or
nursing school faculty members
Hospice or Palliative Care
Program Directors
Hospice or Palliative Care
Organizations or Associations
2. Applicants may apply for more than one
program. However, applicants
must submit a SEPARATE application for each program.
3. Applicants must be recognized leaders in
their institutions or organizations
and must be able to demonstrate their ability to make change
within
their institution or organization.
4. Applicants must provide a letter of
institutional or organizational support.
5. Applications will be accepted from the

APPLICATION
INSTRUCTIONS

The application must be
prepared in the following format:
1. In English only
2. Typed in 12-point font with one-inch margins,
single-spaced, and single-sided
3. Page number and the applicant's full name must
appear in the upper
right-hand corner of every page included in the application
4. Applications may be faxed, emailed, or sent by
air courier (DNL or TNT)
5. Do not send materials other than those
specifically requested in the
application guidelines
6. Please retain copies of all materials submitted
for review, as they will
not be returned.
7. At all stages of the review process, personal
visits as well as telephone calls
to staff by prospective grantees are strongly discouraged.

applications that have

support for the project
16. Specific program related information. If you
are applying for:
PROGRAM 1:
Palliative
Care Resource/Training Center--
you must include a detailed explanation of what your program goals are, how the
goals will be met, what resources will be made available, who will have access
to the resources and training, who the faculty would be, a description of the
training activities, a timetable outlining the project activities and an
explanation of how the program will be evaluated
PROGRAM 2, 3, or 4:
Regional Education Program, Regional Meetings
of Policy Makers, National Education Program--
you must include a detailed explanation of your program goals, a list of the
educational objectives to be reached, a timetable outlining the project
activities. Please provide a "draft copy" of the proposed program and
include the proposed dates, a schedule for the program, a list of faculty (the
use of experts from the country will be favored), a description of the facility
where the program will take place, a list of other funders, the selection
criteria for participants, the number of participants anticipated, and a
description of how the program will be evaluated .
PROGRAM 5:
Palliative Care Scholarship--
you must include a description of the training program you wish to attend, what
your goals are for the scholarship, the dates you wish to attend and a letter of
acceptance from the program
PROGRAM 6:
Palliative Care
Travel Grant-
you must include a explanation of why you wish to attend the conference and what
you believe you will gain from your attendance, a copy of the conference
announcement or the actual program for the conference, the dates of the program,
the cost of registration, and the amount needed for travel and accommodations.
Please note that you will be required to submit a report form following the
conference.
PROGRAM 7:
Education Materials-
you must identify the material to be translated, justify the need for the
translation, identify how the materials will be distributed and to whom they
will be distributed, provide a timetable outlining the work plan, identify who
will do the translation, provide name and contact information for this
individual, explain previous published translation experience, and provide a
two-page sample of the material to be translated both in English and in the
language in which it is to be translated.
