INTRODUCTION: This is a case report of a male patient A.L., 36 years old, affected by bilateral congenital glaucoma. He was operated on, on both eyes, by goniotomy at his birth. In 1978 he was operated, on his RE, by choroidal and retinal detachment. In 1988 he was operated, on his LE, by cyclodiastasis. In 1996 his RE underwent a laser cyclophotocoagulation. Nowadays he got a cataract on both eyes and his IOP, bilaterally, is under control by medical therapy.

DISCUSSION: This is a quite exceptional case report as for its follo-up. Due to this very long follow-up, it is possible to emphasize the history of congenital glaucoma surgery during last 36 years up to now. About our case, we can consider that this patient, even if operated by some skilled surgeons, has a very poor visual outcome.
