Problems during
cataract surgery in eyes with exfoliation syndrome are smal pupils and loose
lenses. Enlargement of the pupil could be performed with stretching, Healon 5
or irisretractors. The combination of exfoliation syndrome and a smal
pupill (without miotic
eyedrops)could mean a loose
crystalline lens. In those eyes irisretractors could be placed in the anterior
capsulorexis to keep the lens in place during surgery. A capsular tension ring
is implanted before the foldable intraocular lens is placed in the capsular
bag.
After surgery eyes
with exfoliation syndrome often develop a pronounced inflammation with high
flare. This could be treated with frequent topical administration of
Dexamethasone.
