CATARACT SURGERY IN EYES WITH
EXFOLIATION SYNDROME

C. Zetterstroem

St Eriks Eye Hospital, Stockholm, Sweden

Problems during cataract surgery in eyes with exfoliation syndrome are smal pupils and loose lenses. Enlargement of the pupil could be performed with stretching, Healon 5 or irisretractors. The combination of exfoliation syndrome and a smal pupill  (without miotic eyedrops)could  mean a loose crystalline lens. In those eyes irisretractors could be placed in the anterior capsulorexis to keep the lens in place during surgery. A capsular tension ring is implanted before the foldable intraocular lens is placed in the capsular bag.

After surgery eyes with exfoliation syndrome often develop a pronounced inflammation with high flare. This could be treated with frequent topical administration of Dexamethasone.
