- Fees for Participants include participation in all scientific
sessions, printed material of the congress, an invitation to the
Get-together Reception, Coffee Breaks as well as light lunch,
entrance to the exhibition and Gala Dinner
- Fees for Students, Fellows and Nurses include participation

- June 16th - Fees for Course participants include course materials
and coffee break.
No lunch will be provided.
** In order to obtain the special fee for Students/Fellows/Nurses,
the registration form must be accompanied by an approval letter
signed by the head of the department (if registered via the website,
please fax the approval).
CANCELLATION POLICY
Up to 90 days prior to arrival - full refund less bank charges
Up to 60 days prior to arrival - cancellation charge of US $50
Less than 60 days prior to arrival - no refund
PAYMENT
Payment of registration fees may be made by cheque payable
to the "7th Headache Congress" or by bank transfer to
the EHF 2004, Credit Suisse Bank Geneva Branch, Switzerland, Account

code: CRESCHZZ 12A, or by credit card, ie: Visa, Mastercard, American
Express or Diners Club.
Please make sure that the name of the congress and the participant
are stated on the bank transfer.
Credit cards are being charged in US dollars at the rate of exchange
on the day of the charge.
LETTER OF INVITATION
Upon request, the Secretariat of the Congress will send
a personal invitation to participants. This invitation is meant
only to help visitors to raise travel funds or to obtain a visa,
and is not a commitment on the part of the organisers to provide
any financial support.
