Our aim was to investigate the effect of DAFNE training on glycaemic control, hypoglycaemia and psychosocial outcomes in Ireland.<br><b><b>Methods: </b> </b>Outcomes included HbA1c and rates of severe hypoglycaemia. Quality of life was assessed using PAID and HADS questionnaires. <br><b><b>Results: </b> </b>Complete data were available for 198 patients (44% male). HbA1c at baseline ranged from 5.1 to 14.2%. HbA1c levels after 1 year showed minimal change from baseline (8.37+/-1.29% vs. 8.31+/-1.36%, p=0.410). <br>Patients with baseline HbA1c &gt;7.5% showed slight improvement in HbA1c 1 year post DAFNE (8.89+/-1.07% vs 8.72+/-1.30%; p=0.051). For the cohort with baseline HbA1c d"7.5%, HbA1c increased at 12 months (6.93+/-0.51% to 7.19+/-0.71%; p=0.007).<br>Rates of severe hypoglycaemia were low at baseline and were not significantly different at 12 months. <br>Mean PAID scores decreased from 22.45+/-17.88 to 14.05+/-13.79, p=0.001. Mean HADS scores decreased from 4.10+/-3.33 to 3.34+/-2.77 (anxiety), p=0.047 and from 3.76+/-2.77 to 3.14+/-2.96 (depression), p=0.097. <br><b><b>Conclusion: </b> </b>These results demonstrate that DAFNE training in a routine clinical setting in Ireland is associated with significant improvement in measures of psychological distress and diabetes-related anxiety. The lack of a significant improvement in HbA1c at 12 months may have resulted from the wide range of HbA1c among participants at baseline. This reflects the policy of the participating centres to offer DAFNE training to all patients with Type 1 diabetes.
